ATTN Members!   It's time to renew the short term disability.  As a reminder this is only for the members
who already have it. They can be faxed to me (Roger) at 324-4080 no cover sheet is necessary. I need the
forms back to me by 5/31/12.  Please ask all members to do the calculations about half way down the page.
I also need to know if there are any members who will not be able to complete the forms due to being
out-sick, military etc. On a good note, Unum has kept our rates the same since the policy's inception
